Hi Donna,
I don't believe there has every been a questionnaire; but it is something that I have always wanted. There has to be a reason why this tumor strikes the adolescent/young adult and around the same age group. Did their parent smoke, did they use soy formula; did they have allergies; did they have projectile vomiting as a baby; blood type of parents; what age was the tumor discovered; what size at the time of diagnosis and had it spread outside the liver; etc. This website would be a good site to establish this database and then forward it to researchers. Also; researchers are going to need tumor samples from everyone to determine common issues too. Duke University Medical Center has a research center that at one time was doing liver studies; but they lost funding. Without a research center and a grant from the NIH, it's really impossible to do anything on our own.
